Have you checked the governor encoder vs the motor encoder. They should be within 5 fpm of each other.

Is it using a tape reader? If so, the guides may be worn to much, giving it to much play.

B and BE are the LEDs. They have to drop and pick together.

Draka makes a Jacket that Velcros around the cable. It's mainly to protect against divider beams but maybe an option for you.

Brake switches dropping in flight, LB relay dropping in flight. Drive losing brake input. All can cause this fault.
