I'm working on a miprom 21 and I have changed the encoder on the motor and on top of the car and I'm still getting faults
142 * 27 28
151 * 27 28
122 *23 13
162 *25 23
Does anyone have any other ideas

Verify that the counts per revelotution. Are the wheel diameters the correct. Are the encoder shafts loose? Look at the way they are mounted. Check the motor coupling. Is it broke or loose? Make sure the car top encoder has spring tension on it. Check your ground lead size is correct. Check your wires and connections.

Then perform a vane reader check on inspection.

comparator troubles, dictated velocity does not match actual velocity. 27. position encoder lost while running. 25. if the car does not have a brake switch, or is not turned on, make sure the brake is picking fully. If that is good, and you have already changed both encoders, then you may have a bad MSU board. if you have a msu board or more then one car in the group try swapping this board.
